Observations and Nurse Escalations

Overview:

Improve the timeliness and visibility of health observations. Escalate potential changes directly to nurses for faster triage and follow-up

Benefits:

  • Better care communication - Make it easier for care staff and nurses to share critical information in real-time
  • Support early interventions - Catch health issues early and track health trends to support proactive reassessments

Roles:

  • Caregivers/CNAs - Capture observations at the point of care. Focus on what you see, smell, or hear. Add notes to describe the observation. Notes will be sent to Nurses to review.
  • Nurses/MedTechs - Get notified of new observations and track them from the Today page. Claim observations promptly and resolve them by the end of your shift after completing any relevant follow-ups.
